After beta, while driving my phone automatically goes into driving mode (which I do not want), and it it me a while to figure out that I was missing notifications because driving mode stays on until I turn if off manually Everytime.
Do you have any bixby routines that are being run automatically when connected to your car Bluetooth ?
it may also be worth checking in other apps like android auto or messaging apps that automatically launch when connected to a Bluetooth device. You may also try turning off Bluetooth on the S21 device next time you are driving to isolate if incident is being triggered due to a BT connection.
