Sleep mode has its own instance of do not disturb. Open sleep mode to edit do not disturb to allow notifications from the clock. Go to the global do not disturb and make sure alarms are allowed in the alarms and sound section. Even though modes snd routines can have separate instances of DND, some things get referenced to the global dnd. In the watch settings make sure to allow notifications from the clock app.
