Pressing "Take All" on the Medication tile doesn't update the S Health app

There appears to be a bug with the medication tile on the Galaxy Watch Ultra. If you tap on the Take All button before the scheduled time, it displays a notification that you have taken your pills, and the tile updates to show when your next pill is due. However, this does not update the app, which does not record that you have taken your medication and will sound an alert at the normally scheduled time.

For instance, I normally schedule my first batch of medication at 8 AM, but I woke up early today and took my pills at 7 AM. I tapped the Take All button on my watch and it acknowledged this and updated the tile to show my next pills were due at 6 PM. But at 8 AM, my phone and watch both sounded an alert that I needed to take my medication.

It also has this same behavior if you snooze the notification, then press the Take All button on the medication tile; when the snooze period is over it will alert you again.

It has done this every single time I have used the Take All button on the tile. Opening the app on the watch and logging the medication there works correctly (though it's a lot of extra steps).

Sometimes it takes a while to sync.  Why not just open the phone app and tap take all when you take your meds early?  Unless it's important for you to know you took your meds at an earlier time, just tap the "take all" when the notification comes up.  If it's important that the time you took the meds be precise, you can go back and edit the time you took the medication in the phone app.

I don't have that problem but mine stopped alerting me on my watch, it only alerts on my phone

Did you make sure you didn't turn off notifications from the health app off in the wear app settings?  You choose your notification categories on the phone, but the notifications have to be allowed in the Wear app.  You may have had the setting on in the past, but something could have caused it to be turned off.  Are you getting other health app notifications on the watch, or just not the medication reminder?  I noticed on my watch sometimes if I get a notification from another app at the same time, I will miss the medication reminder on the watch.  It tends to disappear in five minutes or so if it's not immediately attended to, but it comes back as a repeat (nag), unless you hit "take all" on the phone notification.
