Video crashing with Triple Neo G7 43

I have triple Neo G7 43s attached to a 4090 via DP.  I have game mode turned off.  I also have a generic 4th 32" 1440 monitor I connect to the onboard graphics via HDMI.  5 minutes into a game (any racing game I play) the Nvidia video crashes.  The 4th monitor still works and you can still hear sound from the game.  UIpon rebooting, the Nvidia settings are messed up, for example surround is disabled when it had been enabled, sometimes the 4th monitor will go blank after initial boot.  It takes several attempts to get everything back working by turning settings on and off, rebooting, etc.  Once it all works, it stays working for as long as I remain online.  But if I power everything off (which I always used to do), the issue reoccurs the next time I power up and start playing.

It sounds like it is related to the video cards/driver HOWEVER, I never had this issue when running my previous triple monitors on the same system.  The only thing that has changed is switching from ASUS 32 to the Samsung G7 43.  The 4090 would not even let you connect 3 monitors until Game Mode was turned off, so there definitely seems to be something strange with the monitor.  Looking for help to not have to spend an hour fixing this every time I want to play.

Try turning your refresh rate down to  100 and see how long it lasts.

What was the resolution and refresh on the 32s?

The 32s were 244hz.  I have determined the issue only occurs after I restart after the PC has been powered off.  If I leave the PC plugged in, everything fires up and works without issues.  So it seems the monitors might not be the cause of the problem, albeit I never had a problem on the other monitors when I unplugged everything.
