Wrong app

The wrong application opens when I select an app on my home screen.  The issue is fixed if I clear all apps on the recent menu.  

Anyone else having this problem?

Are you using a 3rd party launcher?

Have you tried clearing the cache partition in the maintenance menu?

Stock launcher and yes to clearing cache partition.
