I have the unlocked model and I don't have that app. I know Sprint used to have it and then Tmobile branded devices. Go to apps, search for it (carrier hub or carrier request), force stop and clear the data and cache

The force stop is greyed out for carrier hub app. I turned off notifications for this app.
