This issue started after the One UI 8 update on my Galaxy A36. I was hearing the ringtone both through my Phonak hearing aids and the phone’s speaker, but before the update it only rang in my hearing aids. This is a known Bluetooth audio routing change that’s affected some Samsung phones after One UI 8. Samsung slightly changed how the ringtone is handled when Bluetooth audio devices (like hearing aids or earbuds) are connected.
I've come up with a fix for it using Modes and Routines. It took many tries to solve it. If anyone else is having the same issue let me know.
