How to remove Bluetooth devices from phone?

I have many Bluetooth devices listed on my phone. Several of them no longer exist. How do I remove them from my phone.?

If they are listed as Paired devices:
Click the Gear icon to the right of one, and click Unpair at the bottom.

If they are listed as Available devices:
These are the Bluetooth devices that are near you, and cannot be cleared until they are either no longer near you, or are not broadcasting any more.  The only way to clear these devices without them being turned off would be to turn off Bluetooth.
