Try creating another instance of Do Not Disturb within a mode and see if that exhibits the same behavior. DND instances in different modes are separate from the global DND and can each have different parameters, so you can have different levels of DND for different situations.
Try creating (or editing an existing mode) that includes do not disturb and configure the DND in that mode the way you want to. You can then turn on the mode manually when you need to or have it on automatically. For instance, sleep mode has its own instance of DND and the default is to let nothing through. I edited that instance to let my family's calls through (because if any one of them calls me in the middle of the night, it's important). You can create a custom mode and then add a widget for that mode to your home screen so you can easily turn it on or off. Settings/modes and routines/add mode. If this alleviates the situation, then keep that mode and use it instead of the global DND.
