Annoying floating mute widget

After an update last week.... i have an annoying floating mute widget that stays on the screen. I move it multiple times while looking at my screen. Sometimes i press it and mutes ALL sounds. It is frustrating. I use my phone for work... and it is frustrating when i accidently press the button while moving it that i will not be able to hear the person on the other line.

How do i get rid of it??????

Open Settings app

Tap Accessibility

Tap Hearing Enhancements

Tap the LEFT side of Mute All Sounds , ie, tap on the word Mute

Mute All Sounds Shortcut is probably enabled
