My Files App - File Location Question

In the My Files App where the Categories are listed (Images, Videos, Document, etc.) The Documents folder contains about 170 items.  I think that is all the document files I have saved.  In the Storage section under Internal Storage, the Documents folder only contains 31 items.

Are the 170 items all stored on my phone or does it includes those that are stored online?  Why are there only 31 items in my Internal Storage, Documents folder and 170 item in the Categories, Documents folder?

Yes, both on the device and on-line.

To further clarify: Going into the categories, when you see the list of files, you can select an item (long press on any file), click on the "more" menu on the bottom right and select details, and you can see the storage location.
