A couple of days ago, my buttons took themselves off-center, only on the cover screen. I couldn't find any setting to realign them to the center. How can I fix this? I found that restarting the phone recenters them, but I don't want to do that every time.

Wiping cache partition should fix this problem.

Thanks for your input but I'm looking for an easier solution for your average phone dummy.

It is an easy solution.
Power off device.
While device is powered off, press the power button and volume up button simultaneously, when you feel a vibration let go of the buttons, you will now be in the bootloader menu, using the volume up and down for scrolling and power button for Yes, scroll to wipe cache partition, scroll to yes, press power button for Yes. Reboot, press power button.

Hey thanks again for your suggestion. Unfortunately, I wiped it twice and it would be normal for like 2 seconds then jump to the side. Got anything else?

Other than a full factory reset, no!

Hello usmctsc7, Welcome to the Community. We understand the frustration of the navigation buttons shifting on the cover screen.

Have you tried testing the phone in Safe Mode to see if an app is shifting the buttons? https://www.samsung.com/us/support/answer/ANS10003495/

Also, try resetting the app settings. Settings>General Management>Reset>Reset all settings

Let us know if there is any change.

Thank you for your advice. Interestingly, booting in and out of Safe Mode appears to have fixed the issue, but I did not need to uninstall any 3rd party apps.

Thanks for following up and letting us know! We are happy to hear that booting in and out of safe helping resolved the button placement. Please let us know if you have any additional questions.

@usmctsc7 Interesting... I'm not seeing that on my Fold 6 cover screen. Have you tried going into Display settings and toggle the button location from right to left and back?
