Galaxy Watch
thanks. it worked after lots of trial errors. following are the steps worked for me
1. uninstall health monitor ( separate app , not the samsung health app) on phone
2. remove old watch from wearable app hardware list ( i updated from watch to watch3 last month, it was still showing in hardware list)
3. reset watch3, restore from backup
4. from watch add widget, ecg click on download app, it installed health monitor app again on phone, now got an option to complete setup on phone
