I get the notification on the watch for phone call. I answer the call from the watch, but the caller can't hear me from the watch and I can't hear the caller through the watch. The caller is heard through the phone and can hear me through the phone.
DND , bedtime, theater are off. Bluetooth connected, Wi-Fi off.
I can use Google Assistant from the watch and hear results through the watch.
Thank you for reaching out. I can certainly understand your concern with your audio not functioning as expected for calls on your Galaxy watch 5 Pro. Just to clarify, if you switch the call from the phone to the watch are you able to hear the caller and are they able to hear you?

I'm having the same problem, I have done a reset multiple times and a factory reset still has the problem. The watch works for a few hours and then I cannot hear anything on incoming or outgoing calls..
